Hours Worked Claim Form
Thank you for working with Orinoco. In order to make your payment faster -- as well as provide an audit trail -- you will need to fill in this form. The data are kept for our files and added to our accounting statements. Unfortunately, we can not accept paper copies as we can not hold paper records.
